5. Egg Retrieval

This shouldn't be a cause for despair, as sperm can be obtained from the husband surgically. It's important to know that sperm is formed in the testes, while mature sperm are stored in the epididymis. Some men's testes can produce sperm, but their semen may not contain any. Therefore, if the husband is found to have azoospermia before the wife undergoes IVF treatment, he will need to undergo surgical sperm retrieval to provide sperm for the IVF process. The husband's sperm can then be cryopreserved for future use by the couple; this type of IVF service is available in Hong Kong.

There are several methods for surgically retrieving sperm. One is percutaneous epididymal sperm aspiration retrieval, in which a fine needle is used to penetrate the scrotal skin directly to the epididymis, extracting a small amount of fluid and retrieving sperm from it. Another method is testicular sperm aspiration retrieval, in which a fine needle is inserted into the testicle, and a small tissue sample is extracted from a tiny channel. This sample is then sent to a laboratory for processing, and sperm is retrieved from the tissue. Both procedures are performed under local anesthesia.

Another procedure is called microscopic epididymal sperm retrieval, which involves making a small incision in the scrotum, clearly separating the tiny tubules of the epididymis under a microscope, and then using a microneedle to extract fluid from the tubules to find sperm. Another method is microscopic testicular biopsy sperm retrieval, which also requires making a small incision in the scrotum, exposing testicular tissue, and then using a microscope to find sperm from the testicular tissue. Both procedures are generally performed under local anesthesia, but general anesthesia/ monitored anaesthesia care may be used if necessary.

It's important to understand that surgical sperm retrieval does not guarantee a 100% success rate for IVF. It depends on the actual ability of the man’s sperm production ability. Even if it is successful to obtain sperms, there is a possibility that no viable sperm will be found after the procedure.
